As soon as you have a confirmation from your insurence that you are covered, you will be covered. I would say 0 second after confirmation.

It depends on which insurance company you are using. Some might not cover pre-existing conditions (assuming you’re asking because of a problem with your wisdom tooth).
If you are shopping around for different plans, read the materials carefully to find out. You can also call and ask. Make sure you write down the name of who you spoke to, the date and what they said, in case there is a problem later.
